We provide this care with a contractual agreement with Insurance companies and other vendors Nursing homes, long term care facilities  and conscier health services with dedicated community members. We also can provide home labs draw, Pt/INR check, ACCU check, Heme check, and  Urine Dip Test. Our Team will coordinate care with the Primary care physicians and specialists.
